Model 1

1. At the end of treatment, you perform range of motion using goniometer or inclinometer

2. You send me the results

3. I calculate the impairment rating, charts, reports.  Your name does not appear on the report, I am an independent doctor

4. I send the reports and an invoice for $1500 to the patient’s attorney

5. I collect the check, I pay you 50% of collections

6. You are a 1099 employee of mine, I am paying you to perform the range of motion for me

Model 2

1.  At the end of treatment, you perform range of motion using goniometer or inclinometer

2. You send me results

3. I calculate the impairment rating, charts, reports.  Your name does not appear on the report, I am an independent doctor

4. I send the reports and an invoice for $100 to you, due upon receipt 

5. You bill the attorney $1500

6. Attorney pays you, you keep the check

7. I am a 1099 employee of you, you are paying me to calculate the impairment and generate the reports on your behalf

Model 3

1.  At the end of treatment, you perform range of motion using goniometer or inclinometer

2.  You send me results

3.  I calculate the impairment rating, charts, reports.  Your name does not appear on the report, I am an independent doctor.

4.  I pay you $100 up front

5.  I bill the attorney

6.  I assume all the risk, you get paid either way
